IMPORTANT MEDICAL INFORMATION
You are responsible to make sure you're medically qualified to participate in this activity. Below you will the medical guidelines & release forms that you will need to fill out when you arrive at the dive shop. No refunds will be given to those deemed medically unqualified. This is for your safety.
Medical History Form	To be completed by all divers joining any of our training programs.
Medical Guidelines & Doctor's Release Form	If you need to visit your physician to establish your eligibility for scuba activities, use this form as a guide for the exam and for your doctor to give you the OK.
